A dental implant acts as an orthodontic anchor. It is a replacement for the root(s) of a tooth, which are secured in the jawbone and are no visible once surgically places. Dental implants are used to secure crowns and other dental prosthetics or dental restorations. Dental implants are made of a lightweight, strong, and biocompatible material called titanium. The fact that it is biocompatible means that there are very low chances of rejection but the body.

The main use of dental implants is to support dental prosthetics. Dental implants make use of a biologic process where bone fuses tightly to the surface of specific materials such as titanium and some ceramics – this is known as osseointegration. The integration of a dental implant and bone can support physical loads for decades without failure. A variable amount of healing time is required for osseointegration before either the dental prosthetic (a tooth, bridge or denture) is attached to the implant or an abutment is placed which will hold a dental prosthetic.

Soft tissue reflection:

An incision if made over the top of the bone, splitting the thicker attached gingiva in half so that the implant that will be placed will be surrounded by a thick bank of tissue (‘flaps’). The flaps are then pushed back to expose the bone. An alternative is flawless surgery, where a small punch of tissue is removed, ready for implant placement.

Drilling at high speed:

Using a surgical guide or stent, pilot holes are placed with precision drills at a regulated speed to prevent burning or pressure necrosis of the bone – a form of cell injury which results in the premature death of cells.

Drilling at low speed:

The pilot hole drilled in step 2 is now expanded by using progressively wider drills at a lower speed. Extreme care is to be taken in order not to damage the osteoblasts or bone cells by overheating while drilling. At the same time, a cooling saline or water spray keeps the temperature low.

Placement of the implant:

The implant screw is placed in one of two ways:

  • The screw can be self-tapping; or
  • The prepared site is tapped with an implant analog, where the implant can be screwed into place with a torque controlled wrench.

Tissue adaptation:

The gum is adapted around the entire implant to provide a think band of healthy tissue around the healing abutment.

Success or failure of implants depends on various factors:

  • The health of the person receiving the treatment;
  • Any medication the patient is consuming, which affect the chances of osseointegration;
  • The health of the tissues in the mouth; and
  • The amount of stress that will be put on the implant and fixture during normal function.

While teeth are susceptible to dental decay, dental implants may be susceptible to peri-implantitis, which is an inflammatory response to bacterial biofilm of the tissues surrounding the implant. If a patient has the ideal bone width, height, and type of bone, a dental implant with a crown can take approximately five to eight months. This includes placing the implant in the bone and waiting for you to heal and the implant to integrate with the bone. After this, we take dental impressions to make the dental crown that will be fitted on the implant. When the crown is ready, the treatment will be finished by fitting the crown on the implant.

If a patient looking to have a dental implant does not have the right type of bone, width or height, they will have to undergo ‘bone grafting’, which is essentially transplanting bone and enhancing the amount that you have so we are able to attach a dental implant. The ‘bone grafting’ process will add an extra four to five months to the process. Including bone grafting, the total time of treatment can be between eight months and a year.

With proper care and attention dental implants can last a lifetime. Like natural teeth, they need to be regularly serviced and maintained to ensure optimum oral care, including healthy gums and a fresh breath.
